A recent prospective, randomized controlled trial demonstrated that a continuous flow (CF) left ventricular assist device (LVAD) resulted in improved survival at 12 and 24months compared to a pulsatile-flow (PF) device. The current study examines the hospitalization costs associated with treatment of New York Heart Failure Class IV patients when implanted with a CF LVAD and compares them to previously reported costs of a PF LVAD in the same population. Strategic investment and product development considerations need to incorporate both reimbursement and patient payment options.how to think about the consumer market in reimbursement strategies. If anyone wants to take a look, you can find it at this link: With the rise of consumer control in healthcare expenditures, many medical technology companies have focused their business strategies around out-of-pocket spending as opposed to third-party reimbursement. Through case studies, we identified three scenarios surrounding consumer-targeted business models. These include: targeting consumer payment for some indications of a product while getting covered by insurers for other indications; targeting consumers solely because of belief that coverage will not be granted; and attempting to get coverage, but forced to pursue out-of-pocket spending after failing. As a result of the recession, companies that rely only on consumers for revenue are threatened, but this does not mean that targeting this market is not a valid choice. Thinking strategically about how your product will be viewed by insurers and consumers and how to market it to these two groups is an extremely important business decision.

On August 8th the Centers for Medicare and Medicaid Services (CMS) issued the Proposed Hospital Outpatient Prospective Payment System (HOPPS) Rule for FY2007:
“Proposed Changes to the Hospital Outpatient PPS and CY 2007 Rates.” [cms.hhs.gov]
In the proposed rule for the Hospital Outpatient PPS, CMS expanded the list of procedures that may be performed in Ambulatory Surgical Centers, added HCPCS codes to better describe hospital emergency visits, reimburse more accurately for drug administration, lowered the reimbursement rate for separately payable drugs to ASP + 5 and added requirements for the reporting of quality measures. More important that the addition of the 14 new ASC codes in 2007, CMS proposed to relax the requirements for a given procedures eligibility to be performed in an ASC to be those that can be done safely in that setting in 2008. ASC payment would be modified to reflect the relative weight calculation of the OPPS to remove site of service bias in facility fee reimbursement. Quorum can help clients understand how the changes to HOPPS may affect them and their customers.

On August 1st the Centers for Medicare and Medicaid Services (CMS) issued the Final Hospital Inpatient Prospective Payment System (HIPPS) Rule for FY2007:
“Medicare Program: Final Changes to the Hospital Inpatient PPS and FY2007 Rates.” [cms.hhs.gov]
In the final Hospital Inpatient PPS CMS scaled back and delayed earlier proposed changes in Medicare payments to hospitals for fiscal year 2007. Based on estimated hospital costs rather than inflated charges, the 2007 payments will have improved accuracy. In the same vein, CMS also updated its method of estimating costs to include more data from high-cost hospitals. The resulting payment changes will be smaller than proposed in April and will be put into effect gradually over three years. Quorum can help clients understand how the changes to HIPPS may affect them and their customers.

The Medicare program provides some guidance; the Office of the Inspector General provides some guidance; and manufacturers are left to sort it out. How can PAPs remain available to patients who need them, and still be compliant with all applicable federal regulations? Quorum participates in this recent CBI conference to present considerations in designing a PAP that makes sense for products in question, and to discuss with industry leaders the nuances and challenges facing manufacturer PAPs today.

PharmacoEconomics is a peer-reviewed journal addressing optimum drug therapy and health outcomes via pharmacoeconomic and quality-of-life assessments. Quorum attends the Current Procedural Terminology (CPT) Editorial Panel Meeting on February 10 through 13 in Newport Beach, California. This panel, which meets at least four times per year, is made up of 17 physicians representing various medical and surgical specialties and third-party payers. The Editorial Panel is responsible for enacting changes to CPT coding language.
